| Medical Benefits | Employees can elect to participate in a PPO medical plan. The plan has a $500 individual deductible and a $15 office visit co-pay for "in-network" services, delivered by a primary care physician. The plan covers prescription drugs, wellness exams, vision exams, and vision materials (frames, lenses and contacts). The company pays a majority of employee-only coverage and assists with the cost of dependent coverage. The company’s
contribution toward the benefits will vary depending on coverage the
employee selects.

| Dental Coverage | The company’s dental plan provides coverage for preventive care (cleanings), basic restorative services (fillings), and major restorative services (bridges

The Healthy Lifestyle Incentive Program is designed to assist employees and covered dependents in identifying and reducing potential health risks. | Retirement Plan | The company contributes four percent of employees’ wages to their retirement accounts. Employees choose how to invest the employer contributions from a list of over 25 mutual funds available through Fidelity Investments. Employees become vested in employer contributions based on their years of service.

| Matched Savings Program | In addition to the company’s contribution to the retirement plan, employees may also contribute toward building their retirement accounts. Employees may contribute up to 75 percent of their annual wages to the savings program. If employees contribute to the Matched Savings Program, the company will match employees’ contributions $0.75 for each $1.00 contributed, up to 6 percent of wages.

| Disability and Life Insurance | The company provides each full-time employee with Long Term Disability, Short Term Disability, and Life insurance. The disability benefits assist in replacing a portion of the employee's wages if the employee is unable to work due to an injury or illness. The company’s life insurance benefit is equal to 150 percent of annual wages. Employees can also purchase additional life insurance for themselves and their spouses.
