Update your workers’ comp forms
Most state-required workers’ compensation forms now call for only the last four digits of the injured worker’s Social Security number. The Texas Department of Insurance, Division of Workers’ Compensation (DWC) revised its forms to ensure the security of confidential information. Employers can click
here to get the forms.

Remember to deliver required notices
When one of your employees misses work due to a job-related injury, you must:

  1. Report the injury to Texas Mutual Insurance Company by submitting DWC Form-1, Employer’s First Report of Injury or Illness. You can also complete the form online.
  2. Give the injured worker a copy of the completed DWC Form-1.
  3. Give the injured worker a Notice of Injured Employee Rights and Responsibilities in the Texas Workers’ Compensation System. The form is available in English and Spanish.

Electronic billing starts January 1
Beginning January 1, 2008, the Texas Department of Insurance, Division of Workers’ Compensation will require health care providers and insurance carriers to exchange billing information
electronically. The change brings workers’ compensation in line with Medicare, group health and other lines of insurance. It also responds to providers’ requests for a streamlined billing process.
